บทคัดย่อ
	การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาผลการดำเนินงานให้บริการดูแลสุขภาพที่บ้านแกผู้ป่วยที่เข้ารับบริการจากโรงพยาบาลนครพิงค์  จังหวัดเชียงใหม่  ซึ่งได้ให้บริการระหว่างวันที่ 1 กันยายน 2537 ถึง 31 ธันวาคม 2539  และศึกษาความคิดเห็นของผู้ป่วย ปัญหาและข้อเสนอแนะเกี่ยวกับการดำเนินงาน ให้บริการดูแลสุขภาพที่บ้าน  รวมทั้งเพื่อหารูปแบบการดูแลสุขภาพที่เหมาะสมแก่ผู้ป่วยและครอบครัว  ประชากรในการศึกษาครั้งนี้คือ ผู้ป่วยที่อยู่ใน 
เขตอำเภอเมือง และอำเภอแม่ริมที่มารับบริการรักษาพยาบาลที่โรงพยาบาลนครพิงค์  และมีความต้องการการดูแลอย่างต่อเนื่องที่บ้าน  จำนวน 131 ราย เครื่องมือที่ใข้ในการศึกษา ได้แก่ แบบสำรวจข้อมูลจากบันทึกและรายงานผู้ป่วย  และแบบสอบถามความคิดเห็นเกี่ยวกับการให้บริการดูแลสุขภาพที่บ้าน 
วิเคราะห์ข้อมูลโดยใข้สถิติเชิงพรรณนา
	ผลการศึกษาพบว่า การดำเนินงานให้บริการ มีผู้ป่วยได้รับการจำหน่ายออกจากโครงการร้อยละ 68.7  ในจำนวนนี้เป็นผู้ที่มีอาการทุเลา และสามารถดูแลตนะองได้  ร้อยละ 55.5 ระยะเวลาที่รับเข้าโครงการ เฉลี่ยรายละ 5.5 เดือน  ผู้รับบริการทุกรายได้รับบริการด้านการส่งเสริมสุขภาพ ร้อยละ
29.1 เป็นบริการด้านการฟื้นฟูสภาพ และร้อยละ 23.7 เป็นบริการด้านการรักษาพยาบาล ผุ้รับบริการ ร้อยละ 64.9 เป็นผู้ป่วยทางอายุรกรรม และเป็นโรคทาง ระบบหัวใจและหลอดเลือดพบมากที่สุด การบริการพยาบาลที่จัดให้มากที่สุดคือ การทำแผล
	ด้านความคิดเห็นเกี่ยวกับการดูแลสุขภาพที่บ้าน  โดยเฉลี่ยมีความเห็นด้วยในระดับสูง (X = 2.74 , SD = 0.20)  ผู้ป่วยและญาติทุกราย มีความพึงพอใจ
ที่ได้รับการคัดเลือกเข้ารับบริการ (X = 3 , SD = 0)  เกิดความรู้สึกอบรมและได้ใกล้ชิดกับครอบครัว (X = 2.97 , SD = 0.17)  เกิดความมั่นใจในการดูแล
สุขภาพตนเองมากขึ้น (X = 2.90 , SD = 0.33)  และสามารถนำความรู้ที่ได้มาปรับใช้ให้เข้ากับชีวิตประจำวันของตนเองได้ (X = 2.77, SD = 0.44)
	รูปแบบการดูแลสุขภาพที่บ้านที่เหมาะสม  จะมีขั้นตอนการดำเนินงานที่ประกอบด้วย พยาบาลประจำโครงการดูแลสุขภาพที่บ้านรับผู้ป่วยจากแผนก
ผุ้ป่วยนอกและผู้ป่วยใน ส่งให้ทีมให้บริการดูแลสุขภาพที่บ้าน  ดำเนินการวางแผนการจำหน่ายผู้ป่วย  แล้วจึงให้บริการดูแลสุขภาพที่บ้าน และติดตามผล
การรักษาหรือส่งต่อเพื่อรับการรักษา  หรือจำหน่ายออกจากโครงการตามสภาวะอาการของผู้ป่วย



Title :Home Health Care Service For Patients At Nakorn Ping Hospital

Abstract

		The purpose of this study were firstly to evaluate home health care 
service for patients at Nakorn Ping hospital in Chiang Mai province. This service had given between September 1, 1994 and December 31, 1996, Secondly, to examine satisfactions, patients and families' problems and suggestions on home health care service, and thirdly, to devise models of home health care to be used for Thai people. The population of this study were 131 patients and their families who received treatment and nursing care from Nakorn Ping hospital, lived in Mueng or Mae Rim district, and required home health care service.  The instrument used for data collection consisted of home health care assessment and questionnaire about patients or families' satisfactions, problems and suggestions on home health care service. The derived data were analyzed by using descriptive statistics.
		The results of this study revealed that home health care service had 
discharged  patients from the project 68.7 %. In these cases, 55.0% of the patients 
improved  their health. The approximate time that patients were admitted for this service 
was 5.5 month per case. Every patients received health promotion, 29.1 % received 
rehabilitation and 23.7 % received nursing care and treatment such as wound dressing.
In this group 64.9 % were medical patients such as coronary heart disease. 
		On the satisfaction of patients and families' with home health care service, the total satisfied were at a high level (X = 2.74,SD = 0.20). Every patients and their families were satisfied  with admission to the home health care service (X = 3, SD = 0), felt warmly and close to their families (X = 2.97, SD = 0.17), convinced of self care (X = 2.90, SD = 0.33) and could adapt this knowledge to their life - style (X = 2.77,SD=0.44).
			To devise a model of home health care service, it  should be compose of a home health care nurse who admits patients from  an out-patient department or in-patient  department, takes them to the home health care team and gives them discharge planning and home health care service with followed up, then referes patients to receive treatment or discharges them from the service, depending on the patient's health.
