Background: There is a large demand gap for home-based medical services. Understanding the
characteristics of the older people in need of services can help provide relevant services more
           effectively and accurately.

Objective: To understand the categories and characteristics of home-based medical services for
           older patients with chronic diseases.

Methods: Based on the cross-sectional study design, from April to November 2021, the health status
of elderly patients with chronic diseases in Chengdu City, Sichuan Province was evaluated by a nurse
using the Chinese version of the Community Health Intensity Rating Scale including four domains
of environment, psychology, physical and health-related behaviors, and scored respectively. At the
same time, the patients were investigated whether they needed home-based medical services.
K-means cluster analysis was used to classify the population with home-based medical services, and
the basic characteristics of patients in different categories were tested by analysis of variance.

Results: A total of 371 patients were evaluated, of whom 191 (51.48%) needed home-based medical
services. The three clusters were 37 (19.37%), 95 (49.74%) and 59 (30.89%), respectively, including those
with excellent health status, prominent psychosocial problems and poor health status. There were
statistically significant differences in education level, economic status, health status, living status and
cohabitation status between the two groups (P< 0.05).

Conclusion: The health status of elderly patients with chronic diseases in the community is different
with different education levels, economic status, living conditions and family support. In the future,
different levels of services, including health promotion, psychosocial intervention and comprehensive
improvement of health deficit status, are needed for patients with different health levels.
