Development of Clinical Nursing Practice Guideline for Preventing Ventilator
Associated Pneumonia in Intensive Care Unit of Somdet Phra Sangharaja 19

Background: Most crisis patients in the intensive care unit (ICU) need a mechanical ventilator to treat
respiratory disorders or support their respiratory function. Based on the literature review, although
many clinical nursing practice guidelines (CNPG) for preventing ventilator-associated pneumonia
(VAP) have been developed, VAP still has been found and caused severe complications for crisis
patients.
Objective: To develop a CNPG for preventing VAP and examine the effect of using this CNPG on the
VAP rate within seven days in the ICU of Somdet Phra Sangharaja 19 Hospital.
Methods: Soukup’s evidence-based nursing model developed in 2000 was used as a conceptual
framework for developing this guideline. Five from 23 studies were selected and apprised using Joanna
Brigg Institute’ level of evidence. The range of levels of evidence was from 1c to 2c. The developed guideline
was assessed by five experts using AGREE II instrument and its result was 94.20%. Quasi-experimental
designs (One-group posttest design) was used to test the effect of the developed CNPG on VAP rate
within 7 days after intubated tube with a ventilator support. Samples were consisting of four patients
and ten nurses who used this CNPG. Data were collected using a hospital monitoring form and nurses’
satisfaction questionnaire and were analyzed using descriptive statistics.
Results: four patients using this CNPG did not develop VAP within 7 days. No chest x-rays of these
patients during the 24- to 48 hours were changed. The nurses’ satisfaction with using this CNPG was
high (X = 4.12, SD=1.33)
Poster Presentation Abstracts
Conclusion: This CNPG should be used to prevent VAP complications for crisis patients with mechanical
ventilator in the ICU. However, further development is needed to ensure the safety and good outcome for
the crisis patients.
