Background: As global aging intensifies, the proportion of older adults with dementia is increasing
significantly. Care risks for the older adults with dementia happened frequently. The measuring the
care risks in older adults with dementia are both diverse and complex.

Objective: The aim of this study is to identify and evaluate existing assessment scales for care risk
associated with the older adults with dementia, to provide a comprehensive overview of
evidence-based practices and highlight notable gaps in the current research landscape.

Methods: This study was conducted in adherence to the Joanna Briggs Institute (JBI) methodology
for scoping reviews, alongside the PRISMA checklist designed for such reviews. A systematic search of
9 databases (PubMed, CINAHL, Web of Science, SCOPUS, MEDLINE, Cochrane Library, Sinomed, China
Knowledge Network, Wanfang, and Weipu) was conducted from inception until September 30, 2023.

Results: The review identified 13,106 articles, 11 met the inclusion criteria. Several measurements were
recognized: some assess care risk for older adults with dementia residing at home and others for
long-term care facilities. The conceptual frameworks of these tools were generally unclear,
complicating the interpretation and application of their findings. Further details regarding the types of
           studies included, predominantly observational.

Conclusion: While several multidimensional care risk scales have been developed, particularly for
home care for the older adults with dementia, there is a distinct lack of measurements designed for
long-term care facilities. There is a critical need for further development of comprehensive,
multidimensional care risk assessment measurements. Such measurements would enhance
dementia caregivers’ ability to assess and manage care risks effectively, thereby reducing overall
           care-related risks.
