Background: Stroke patients experience long-term impacts and require behavioral changes to
prevent recurrence and reduce disability. However, most stroke patients believe that once they have
passed the critical phase, their bodies can return to normal. This misconception leads to a lack of
awareness about the need for behavioral changes, causing them to continue with their previous
lifestyles, which increases the risk of recurrence. Therefore, nurse practitioners play a crucial role in
fostering appropriate awareness in stroke patients. They design care plans based on the Health Belief
Model (HBM) to enhance patients’ perceptions of disease severity, risk susceptibility, the benefits of
behavioral changes, and barriers to action, ultimately enabling patients to adopt appropriate health
           behaviors.

Objective: To design and develop innovations to enhance awareness based on the Health Belief
Model (HBM) for stroke patients, leading to appropriate behavioral changes.

Methods: Nurse practitioners conduct a comprehensive health assessment and analyze factors
based on the patients’ context and lifestyle.

Design and develop innovations to enhance awareness based on the Health Belief Model for stroke
patients. These innovations include the use of videos, blood pressure assessment results using the
“Seven-color balls” model, and brain CT scan results, combined with the “Reflection Mirror” innovation.
Implement the awareness enhancement plan based on the Health Belief Model. Follow-up and
evaluate health outcomes in the short term, intermediate term, and long term.

Results: 1. Short-term outcome: Patients develop appropriate awareness.
           2. Intermediate outcome: Patients can adopt suitable self-care behaviors.
           3. Long-term outcome (after one month): Patients show improved blood pressure levels (from orange
to yellow) and facial lesion symptoms return to near-normal condition.

Conclusion: Enhancing awareness in stroke patients through various processes and innovations is
crucial for promoting appropriate self-care behaviors. This leads to a reduction in the risk and severity
           of the disease.
